Record as first-class component
Every clinical decision captures the frame, the candidates considered, the evaluation scores, the gate action, and any human judgment. The record is built to be read by someone not present at the decision: a reviewer, an attorney, a regulator.
Edge of the standard
When a case fits no standard category, the system recognizes it has reached an edge and routes to a named clinician. This is not a failure. It is designed escalation for cases that require human judgment.
Governing edge: Harm threshold
The system applies a harm-threshold gate. If the potential consequences of being wrong exceed a configured threshold, the case escalates regardless of confidence. Better to escalate a straightforward case than to miss an atypical one.

Our founder built the business intelligence practice at Accenture reaching 400+ hospitals. He scaled Vizient's BI business to 900+ hospital clients, later acquired into IBM Watson Health. He built and exited Total Benchmark Solution to Kaufman Hall and Madison Dearborn in 2016.
The process methodology was proven at Mayo Clinic, Cleveland Clinic, Stanford Health Care, Ascension, and hundreds of other health systems. That methodology is now formalized into OrbisFramework.
